In 2001 a board of inquiry issued 52 recommendations and 2 additional supplementary recommendations for implantation across all Groups and Services within Defence. While the recommendations were being implemented action was deemed to be slow. As a result it was intended that the recommendations would be built into the safety strategic plan and monitored by the highest safety committee in Defence. It was the CSI task to integrate the outstanding recommendations into the strategic plan and determine the methods for monitoring the quality of the implementation plans.
